• Schedule: Mon-Fri; 7a-3p (flexible)
• Rate: Travel: $2,028/40-hours Local: $1,952/40-hours
Requirements:
• Active Pennsylvania RN license
• Nurse Compact / Multi-state license (required)
• Minimum 3+ years of hospice care experience
• Must have their own TABLET (smart phone does not work with their software)
• Proven clinical experience in care coordination or case management (preferred)
• Candidates must have reliable transportation and be willing to travel to/from patient homes and facilities
• Strong communication, critical thinking, and organizational skills
• Case Manager assignments (field only, mileage reimbursed)
Responsibilities:
• Develop, implement, and modify care plans based on patient needs
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Document all interactions and updates in accordance with facility standards
• Ensure compliance with state regulations, professional guidelines, and organizational policies
• Maintain confidentiality and patient data security in a remote environment
