Schedule: Days, 8-hour shift (9:00 AM – 5:30 PM)
Weekend Requirement: Rotating weekends (approximately 4 weekends per year)

Position Summary:
Provide outpatient physical therapy services in accordance with facility policies, state licensure, and professional standards. Deliver patient assessments, treatment plans, therapeutic interventions, documentation, and discharge planning. Complete required competencies and collaborate with interdisciplinary team to ensure quality care.

Minimum Requirements:

• Current PT (Physical Therapist) licensure
• Bachelor’s degree (minimum)
• Basic Life Support (BLS) certification (must be obtained within 1 month of hire or transfer)
• Successful completion of new-hire competency assessments

Compensation & Benefits:

• Sign-on bonus: $20,000 with a 2-year commitment

Additional Notes:

• Standard workday is 9:00 AM–5:30 PM.
• Position requires adherence to facility onboarding and competency verification processes.
