Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Bronx, NY)

Join a value-based primary care team in the Bronx serving older adults in a hybrid role that combines outpatient clinic visits with home-based care. This locum-to-perm position converts to permanent within 3–6 months and offers guaranteed full-time hours.

Compensation: $75–$95/hour

Schedule and Patient Load

• Full time, Monday–Friday (5 days/week) with guaranteed hours
• Clinic: small daily census of 7–10 patients
• Home visits: 3–5 patients per day
• Low daily patient volume to allow for thorough, patient-centered care

Key Responsibilities

• Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and acute problem treatment
• Diagnose, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging) and manage follow-up care
• Document accurately in the EHR and maintain compliance with regulatory requirements
• Educate patients and caregivers on wellness and self-management
• Collaborate with physicians, nurses, and interdisciplinary staff; coordinate specialist referrals and transitions of care

Qualifications

• Active NP or PA license in New York
• National board certification
• Minimum 2 years of primary care experience (geriatric experience preferred)
• Medicaid and Medicare provider ID required
• Spanish speaking preferred
• Expedited credentialing available
