Locums-to-Perm Nurse Practitioner or Physician Assistant — Geriatric Primary Care (Hybrid)

Join a value-based primary care team serving older adults in a hybrid locum-to-perm role (transition expected within 3–6 months). This position blends outpatient clinic shifts with home-based visits, providing longitudinal, patient-centered care across Bronx, Brooklyn, and Ridgewood, NY.

Compensation: $75–$95 per hour

Schedule & Caseload

• Full-time, Monday–Friday (hours guaranteed)
• Clinic: small panel, typically 7–10 patients/day
• Home visits: 3–5 patients/day
• Low daily census and focused, comprehensive encounters

Key Responsibilities

• Provide full-scope primary care for geriatric patients, including preventive care, chronic disease management, and acute interventions
• Diagnose, treat, prescribe, and create individualized care plans
• Order and interpret labs, imaging, and other diagnostics
• Accurately document care in the EHR and manage follow-ups and specialist referrals
• Educate patients and promote wellness and care coordination
• Collaborate with physicians, nurses, and support staff to deliver high-quality, compliant care
• Adhere to state, federal, and professional regulatory requirements

Qualifications

• Active NP or PA license in New York
• National board certification
• Minimum 2 years’ primary care experience (geriatric experience strongly preferred)
• Medicaid and Medicare provider IDs required
• Spanish language skills preferred
• Expedited credentialing available
