Locum-to-Perm Nurse Practitioner/Physician Assistant – Geriatric Primary Care (Hybrid, Queens, NY)

Locum-to-perm opportunity (conversion within 3–6 months) for an experienced Nurse Practitioner or Physician Assistant in a value-based primary care practice serving geriatric patients. Hybrid role combining outpatient clinic coverage and home-based patient visits.

Compensation

• $75–$95 per hour

Schedule & Clinical Volume

• Full time, Monday–Friday (5 days/week)
• Guaranteed hours
• Clinic: 7–10 patients/day (low census)
• Home visits: 3–5 patients/day

Setting

• Primary care for elderly patients in a value-based care model
• Hybrid: outpatient clinic + home-based care

Key Responsibilities

• Provide comprehensive primary care for geriatric patients: preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and create individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging)
• Document accurately in the EHR
• Educate patients and support wellness initiatives
• Coordinate care with physicians, nurses, support staff; manage referrals and follow-ups
• Maintain compliance with state, federal, and professional regulations

Minimum Requirements

• Active NP or PA license in New York
• National board certification (NP or PA)
• Minimum 2 years’ primary care experience
• Medicaid and Medicare provider ID required
• Spanish language skills preferred
• Expedited credentialing available
