Hospice RN Case Manager (Contract/Travel) — Barre, VT — 36-hr Week, ASAP Start

Join a contract/travel opportunity in Barre, VT as a Registered Nurse providing hospice case management. This role starts ASAP, follows a Monday–Friday schedule (36 hours/week), and offers $2,308 per 36-hour week.

Requirements:

• Active RN license in Vermont.
• 1–2+ years of clinical nursing experience, ideally in hospice, palliative care, or home health.
• Strong patient assessment, clinical judgment, and communication skills.
• Working knowledge of hospice regulations, standards, and individualized care planning.

Key Responsibilities:

• Conduct comprehensive assessments and develop, implement, and evaluate individualized hospice care plans.
• Coordinate care across interdisciplinary teams, including physicians, nurses, social workers, and other providers.
• Educate and support patients and families on disease progression, symptom management, and end-of-life decision-making.
• Maintain accurate, timely documentation in accordance with hospice standards and regulatory requirements.
• Monitor patient status and revise care plans as needed to ensure optimal comfort and quality of life.
• Participate in interdisciplinary team meetings and contribute to quality improvement efforts.
