Required qualifications:

• Minimum 1 year PT experience; new graduates accepted if they have Level II clinical experience (Level I observation-only not acceptable)
• Experience in long-term care (LTC) or rehab
• Active Missouri physical therapy license; Compact licenses accepted

Responsibilities (typical):

• Provide skilled physical therapy evaluation and treatment in LTC/rehab setting
• Develop and implement individualized treatment plans, document progress, and communicate with interdisciplinary team
• Follow facility policies, state regulations, and payer documentation requirements

Travel/contract expectations:

• Temporary placement beginning 06/15/2026 for the specified schedule and rate
• Must meet credentialing and onboarding requirements prior to start date
