Requirements:

• Active, valid Licensed Practical Nurse state license.
• Current BLS certification.
• Registry personnel must bring state or federal photo ID and wear a photo badge at all times.
• Agency staff must have completed original or updated COVID-19 vaccination series or present documented medical/religious exemption from employer. Additional doses required where mandated by law.

Primary responsibilities:

• Provide direct nursing care under RN or physician supervision.
• Perform patient assessments and administer medications and treatments per orders.
• Evaluate, clean, dress, and manage wounds; monitor for infection/complications.
• Continuously monitor and document vital signs and patient condition.
• Maintain accurate patient records and provide patient/family education on conditions and self-care.
• Respond promptly to medical emergencies and follow infection control protocols.
