Requirements:

• Active Licensed Practical Nurse (LPN) state license.
• Current BLS certification.
• Registry personnel must bring state or federal photo ID to the facility and wear a photo badge at all times.
• Agency staff must have completed the original or updated COVID-19 vaccine series or present a documented medical or religious exemption from their employer. Additional doses must be obtained where required by law.

Key Responsibilities:

• Provide direct nursing care under the direction of RNs or physicians.
• Conduct patient assessments and administer prescribed medications and treatments.
• Perform wound care: evaluate, clean, dress, and monitor for infection or complications.
• Monitor and document vital signs and patient status continuously.
• Maintain accurate, timely patient records and provide patient/family education on conditions and self-care.
• Respond promptly in medical emergencies and follow infection control and facility policies.
