Travel RN Hospice Case Manager — White River Jct, VT (36-hr Contract)

Join a compassionate team in Hartford (White River Jct), VT as a contract Registered Nurse working hospice case management. This daytime role starts ASAP, is a 36-hour/week assignment, and pays $2,560 per 36 hours.

Who we’re looking for

• An RN with an active Vermont license.
• At least 1–2 years of nursing experience — hospice, palliative care, or home health experience is preferred.
• Strong clinical assessment skills, excellent communication, and comfort working independently in the community.
• Familiarity with hospice regulations, standards, and care planning.

What you’ll do

• Conduct thorough assessments and develop, implement, and evaluate individualized hospice care plans.
• Coordinate care with physicians, nurses, social workers, and other members of the healthcare team.
• Educate and support patients and families about disease progression, symptom management, and end-of-life decisions.
• Maintain accurate documentation that meets hospice standards and regulatory requirements.
• Monitor patients’ conditions, updating care plans as needs change.
• Participate in interdisciplinary team meetings and help drive quality improvement efforts.
