Occupational Therapist (Contract/Travel) — Hayward, CA

Join a dynamic contract team in Hayward, CA, providing occupational therapy services for a 13-week assignment beginning ASAP. This full-time role follows a Mon–Fri schedule, 8:00 AM–4:30 PM. Compensation: Travel rate $2,308/36 hrs; Local rate $2,275/36 hrs.

Requirements:

• Active, valid Occupational Therapy state license
• CPR and BLS certification from the American Heart Association or Red Cross (AED included)

Key Responsibilities:

• Assess patients’ physical, cognitive, and environmental needs to identify functional limitations and establish measurable goals
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
• Implement evidence-based therapeutic interventions to improve daily living skills and promote independence
• Recommend, train in, and fit adaptive equipment and assistive devices
• Provide hands-on training and coaching for activities of daily living (ADLs) and instrumental ADLs
• Educate patients and families on condition management, safety, and strategies to maximize independence
• Monitor, document, and adjust treatment plans based on ongoing progress and outcomes
• Coordinate care with multidisciplinary providers to support successful community and workplace reintegration
• Maintain accurate, timely clinical documentation including evaluations and progress notes
• Stay current with occupational therapy best practices and participate in professional development
• Perform additional duties as assigned by management
