RN Case Manager Home Health (Full-Time, Days) — VT & NH Sites

What you’ll do

• Complete patient screening, initial evaluations, and periodic re-evaluations.
• Develop and update individualized care plans in collaboration with the patient, family or caregivers, and the patient’s physician.
• Coordinate care through clear communication with the interdisciplinary team and by participating in case conferences when needed.
• Monitor patients’ clinical, functional, and emotional status; identify problems or progress and communicate significant changes to the physician and VNA colleagues.
• Document assessments, treatment goals, and interventions accurately and in a timely manner.
• Teach patients and caregivers self-care techniques and provide medication, diet, and health counseling as ordered by the physician.
• Notify team members promptly about discharges and other key transitions of care.

Who we’re looking for

• Licensed Registered Nurse in Vermont and New Hampshire, or holder of an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Minimum of 2 years’ home health experience.
