As an RN Case Manager with Visiting Nurse and Hospice for Vermont & New Hampshire, you’ll build strong relationships with patients, work closely with an interdisciplinary team, and coordinate care that helps people remain comfortable and independent at home.

What you’ll do
- Complete patient screening, initial evaluations, and periodic re-evaluations.
- Develop and update individualized care plans in collaboration with the patient, family or caregivers, and the patient’s physician.
- Coordinate care through clear communication with the interdisciplinary team and by participating in case conferences when needed.
- Monitor patients’ clinical, functional, and emotional status; identify problems or progress and communicate significant changes to the physician and VNA colleagues.
- Document assessments, treatment goals, and interventions accurately and in a timely manner.
- Teach patients and caregivers self-care techniques and provide medication, diet, and health counseling as ordered by the physician.
- Notify team members promptly about discharges and other key transitions of care.
Who we’re looking for
- Licensed Registered Nurse in Vermont and New Hampshire, or holder of an unencumbered compact license.
- Graduate of an accredited school of nursing.
- Minimum of 2 years’ home health experience.
