Job Summary:
We are seeking a compassionate and patient-centered Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing adult base primary care team. The ideal candidate will provide comprehensive, high-quality care to patients across the lifespan, focusing on health promotion, disease prevention, and chronic disease management in an outpatient setting.
Position Details:
- Provide comprehensive primary care services, including assessment, diagnosis, and treatment of acute and chronic conditions to patients 14 years and above
- Perform physical examinations and interpret diagnostic tests and lab results
- Develop and implement individualized treatment plans
- Prescribe medications in accordance with state regulations and clinical protocols
- Deliver preventive care, including wellness exams, immunizations, and health screenings
- Educate patients and families on disease prevention, health maintenance, and treatment plans
- Manage chronic conditions such as diabetes, hypertension, and asthma
- Collaborate with physicians and interdisciplinary care teams to ensure coordinated care
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in quality improvement initiatives and population health management
- Adhere to clinic policies, procedures, and compliance standards
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current and unrestricted NP or PA license in the state of practice
- National board certification (AANP/ANCC for NP; NCCPA for PA)
- Current DEA license or ability to obtain
- Basic Life Support (BLS) certification (ACLS preferred)
- Minimum of 1–2 years of clinical experience in primary care preferred (new graduates may be considered)
