Family Medicine Float Nurse Practitioner or Physician Assistant — Abbeville, SC (Multi-Site)

We’re looking for a dedicated Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating provider. In this role you’ll deliver patient-centered primary care across several outpatient sites serving Abbeville, Greenwood, Laurens, McCormick, and Saluda counties in South Carolina. You’ll play a key role in preventive care, chronic disease management, and building strong relationships with patients and their families.

What you’ll do:

• Provide comprehensive primary care for patients of all ages, focusing on wellness, prevention, and chronic condition management.
• Travel regularly between assigned family medicine clinics in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Deliver evidence-based care and coordinate treatment plans in collaboration with physicians, nurses, and other team members.
• Teach patients and families about diagnoses, medications, and healthy lifestyle choices.
• Accurately document patient encounters in the electronic health record in a timely manner.

What we’re looking for:

• Graduation from an accredited Nurse Practitioner (APRN/NP) or Physician Assistant program.
• Minimum of 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license.
• Certification or eligibility for certification in your specialty (FNP, PA-C).
