Adult Geriatric Nurse Practitioner — Family/General NP (Mesa, AZ)

Full-time position in Mesa, AZ providing comprehensive primary care for adult and geriatric patients. Manage acute and chronic conditions, perform assessments, develop treatment plans, prescribe medications, coordinate care, and document in the EHR. Deliver culturally competent, patient-centered care and work independently while collaborating with the care team.

Required qualifications

• Graduate of an accredited Nurse Practitioner program
• Active Arizona RN license and Arizona Advanced Practice (NP) license
• Nurse Practitioner certificate from the Arizona Board of Nursing
• Valid DEA registration
• Current Basic Life Support (BLS) certification; CPR/AED per AHA or Red Cross
• Valid Arizona driver’s license
• Valid Level One Fingerprint Clearance Card (required for behavioral health sites)

Experience and skills

• Experience treating adults with acute and chronic conditions; experience in geriatrics or managing multiple chronic conditions preferred
• Proficient in clinical assessment, diagnosis, treatment planning, and medication management
• Ability to work autonomously and make sound clinical decisions
• Strong interpersonal and communication skills with patients, families, and colleagues
• Basic proficiency in business English, spelling, and punctuation
• Comfortable using electronic health record systems and basic computer applications
• Commitment to patient-centered care and ongoing professional learning
