- Active Hawaii PT license required
- Minimum 2 years as a licensed Physical Therapist
- Minimum 1 year home health PT experience
- Experience with Homecare Homebase and OASIS required
- Productivity expectation: 25 points per week
- CPR certification required
- Bachelor’s degree required; Master’s preferred
- Travel assignment experience preferred
Other:
- Schedule is day shift only; no weekend work requested.
