Required Qualifications:
- Active, unrestricted RN license in Colorado
- Minimum 2 years RN experience
- Basic Life Support (BLS) certification
Core Responsibilities:
- Assess patients and identify individualized goals of care
- Provide symptom management as palliative care specialist and generalist for all diagnoses
- Develop and manage an assigned patient caseload; lead and coordinate the interdisciplinary team
- Perform high-quality clinical assessments and apply critical thinking to care planning
- Educate and support family caregivers to build confidence in home care
- Deliver care in patients’ homes, nursing homes, group homes, and assisted living facilities
