CDTM AmCare Clinical Pharmacy Specialist needed at large health system near NYC CPH# JOB-3321968

Seeking an Clinical Pharmacy Specialist - Collaborative Drug Therapy Management (CDTM) for a teaching hospital in the greater New York metropolitan area. This ambulatory care-focused role works collaboratively with physicians to manage chronic disease states through medication optimization, laboratory monitoring, prescribing within approved protocols, patient education, and clinical documentation. Full-time Monday-Friday day shift schedule with no weekends required. Competitive compensation of $160,000-$168,000 plus comprehensive benefits, retirement matching, tuition reimbursement, generous PTO, and potential sign-on bonus.

Candidates must possess an active New York pharmacist license, PharmD degree, required CDTM certification, and completion of a PGY1 residency. Applicants must also have either a PGY2 Ambulatory Care Residency or at least two additional years of ambulatory care pharmacist experience.
