• Minimal call schedule shared among providers statewide
• RN call center handles calls, only urgent cases reach physician
• See 10 - 12 patients daily in senior primary care
• Value-based care experience preferred
• eClinicalWorks and HEDIS experience valued
• Board-certified or board-eligible required
