Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Bronx, NY)

Locum-to-perm opportunity (convert 3–6 months) with a value-based primary care practice serving geriatric patients. Hybrid role combining outpatient clinic and home-based visits.

Compensation

• $75–$95 per hour

Schedule

• Full time, Monday–Friday (5 days/week)
• Guaranteed hours

Patient Volume

• Clinic: 7–10 patients/day (low census)
• Home visits: 3–5 patients/day

Setting

• Hybrid (outpatient clinic + home-based care)
• Focus on value-based primary care for elderly patients

Core Responsibilities

• Provide comprehensive primary care for geriatric patients: preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging)
• Accurately document care in the EHR
• Provide patient education and promote wellness
• Coordinate care with physicians, nurses, support staff, and specialists; manage referrals and follow-up
• Maintain compliance with state, federal, and professional regulations

Qualifications

• Active NY NP or PA license
• National board certification (NP or PA)
• Minimum 2 years’ primary care experience
• Medicaid and Medicare provider ID required
• Spanish language skills preferred
• Expedited credentialing available
