Float Family Nurse Practitioner — Family Medicine (Phoenix, AZ)

Full-time position based in West Phoenix, AZ. Provide versatile primary care across multiple clinic sites as a float Family Nurse Practitioner, delivering patient-centered assessment, diagnosis, treatment, and follow-up in outpatient primary care settings.

Responsibilities

• Provide comprehensive primary care for patients of all ages: history-taking, physical exams, ordering/interpreting labs and imaging, diagnosis, treatment, and medication management.
• Manage acute and chronic conditions common to family medicine.
• Float between clinics as assigned, adapting workflow to different clinical environments and teams.
• Coordinate care and communicate effectively with physicians, specialists, nursing staff, and ancillary services to ensure continuity.
• Provide patient education, preventative care counseling, and develop follow-up plans.
• Maintain accurate, timely medical records and comply with clinic policies and regulatory requirements.

Requirements

• Master’s or Doctoral degree in Nursing with Family Nurse Practitioner focus.
• Current FNP certification.
• Active Arizona RN/APRN license and DEA registration.
• Experience in family medicine or primary care preferred.
• Strong clinical assessment, diagnostic, and communication skills.
• Ability to adapt quickly to different clinical settings and workflows.
