Primary responsibilities

• Assess patients’ physical and cognitive abilities and environmental barriers to identify functional limitations and set measurable goals
• Develop and implement individualized treatment plans using evidence-based therapeutic techniques to improve activities of daily living
• Recommend and instruct patients on adaptive equipment and assistive devices
• Educate patients and families on strategies to maximize independence and manage conditions
• Monitor, document, and communicate patient progress and collaborate with the multidisciplinary care team
