Float Nurse Practitioner or Physician Assistant — Family Medicine (Abbeville & Surrounding SC Counties)

We are hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to serve as a floating primary care provider across outpatient family medicine clinics in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties (SC).

Responsibilities

• Provide comprehensive primary care across the lifespan: acute visits, chronic disease management, wellness visits, preventive care, and medication management.
• Travel regularly between assigned family medicine sites within the five-county region.
• Deliver evidence-based care, emphasizing prevention and chronic condition control.
• Coordinate care with physicians, nursing staff, and ancillary team members.
• Educate patients and families on diagnoses, treatment plans, medications, and self-care.
• Complete timely, accurate documentation in the electronic health record.

Requirements

• Graduate of an accredited NP or PA program.
• Minimum 2 years’ experience in primary care/family medicine.
• Active, unrestricted South Carolina license (or eligible).
• National certification appropriate to role (FNP, PA-C) or eligibility.
• Valid driver’s license and reliable transportation for travel between sites.
