Qualifications

• MD or DO from an accredited medical school
• Completed ACGME‑accredited residency in Family or Internal Medicine
• Board certified or board eligible in Family or Internal Medicine
• Active, unrestricted Arizona medical license (or eligible)
• Current DEA registration (or eligible)
• Prior family medicine experience preferred
