Locums-to-Perm NP/PA – Geriatric Hybrid Primary Care (Queens, NY) $75–$95/hr, Low Census & Home Visits

Locum-to-perm opportunity (conversion expected in 3–6 months) for an experienced Nurse Practitioner or Physician Assistant to join a value-based primary care practice serving geriatric patients in Queens, NY. Hybrid role with outpatient clinic and home visit responsibilities.

Compensation

• $75–$95 per hour

Schedule & Patient Volume

• Full time, Monday–Friday (5 days/week), hours guaranteed
• Clinic: ~7–10 patients/day (small census)
• Home visits: ~3–5 patients/day
• Low daily patient census overall

Setting

• Hybrid: outpatient clinic + home-based care
• Value-based primary care focused on elderly patients

Key Responsibilities

• Provide comprehensive primary care for geriatric patients: preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging)
• Document accurately in the EHR
• Provide patient education and promote wellness
• Coordinate care with physicians, nurses, support staff; refer to specialists and manage follow-ups
• Maintain compliance with state, federal, and professional regulations

Requirements

• Active NY NP or PA license
• Minimum 2 years primary care experience
• National board certification
• Medicaid and Medicare provider IDs required
• Eligible for expedited credentialing
