• Board Certified in Otolaryngology or truly Board Eligible (0–5 years post-residency) — required

• Active NPDB Self-Query Report dated within 30 days at presentation — required (no exemptions)
• Willingness to obtain an active DEA registration prior to start — preferred
• Clean malpractice and licensing history; license and disciplinary history documentation required
• Visa status: indicate if J-1 or H-1B support is required in submission — required
