Job Description
We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our growing Post-Acute Care team in Springfield, MA. In this full-time, facility-based role you will provide high-quality, patient-centered care to individuals transitioning from acute hospital care to skilled nursing and rehabilitation facilities. Working closely with interdisciplinary teams, you will manage chronic and acute medical conditions, support recovery, reduce hospital readmissions, and help optimize patient outcomes in the post-acute setting.
Key responsibilities:
- Conduct facility rounds, perform assessments, develop and adjust treatment plans, and coordinate care with nurses, therapists, families, and physicians
- Manage acute issues and chronic disease, prescribe and monitor medications, and facilitate timely transitions of care
- Document clinical encounters and care plans accurately in the EMR and participate in quality improvement initiatives
- Build long-term relationships with patients and care teams and support patient- and family-centered care
Position highlights:
- Full-time, Monday–Friday, 9:00 AM–5:00 PM; no weekends or on-call responsibilities
- Facility-based rounding in post-acute care settings

Compensation & benefits:
- Base salary: $120,000 annually; total earning potential: $180,000–$220,000+

Requirements:
- Graduate of an accredited NP or PA program and national board certification
- Current, unrestricted NP or PA license in Massachusetts or eligibility for licensure; DEA registration or eligibility to obtain
- Minimum of one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities; ability to work independently while collaborating within a multidisciplinary team
- Proficiency with EMR systems
