Job Description
Join our expanding Post-Acute Care team on Cape Cod, providing patient-centered care for individuals transitioning from acute hospitals to skilled nursing and rehabilitation facilities. This full-time, facility-based role focuses on managing chronic and acute conditions, supporting recovery, reducing readmissions, and optimizing outcomes through close collaboration with interdisciplinary teams.
Locations: Barnstable, MA (primary); additional openings in Wakefield, RI and North Conway, NH.
Schedule & Work Environment
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends or on-call responsibilities
- Facility-based rounding in post-acute care settings
- Clinical autonomy with strong operational support
- Opportunity to develop long-term relationships with patients and care teams
Compensation & Benefits
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

Qualifications
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure
- National board certification (NP or PA)
- DEA registration or eligibility to obtain
- Minimum one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities
- Ability to work independently while collaborating effectively within multidisciplinary teams
- Proficiency with electronic medical record (EMR) systems
