Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing Facilities, Brooklyn (FT)

Provide high-quality, patient-centered care to residents in skilled nursing facilities across Brooklyn, NY, within a collaborative, team-based environment. Openings also available in IL, MI, NH, NC, OH, and WI.

Schedule and duties

• Full-time with flexible scheduling
• Six-hour facility shifts for full-time providers (rounds typically start between 7:00–10:00 AM)
• No call or overnight responsibilities
• Perform daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans
• Document efficiently using streamlined EHR with AI scribe, voice dictation, and company-provided iPad with LTE
• Access UpToDate® for clinical guidance

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Relevant board certification (ANCC, AANPCB, PNCB, AACN, NCCPA as applicable)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
