Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing (Ossipee, NH)

Provide high-quality, patient-centered care to older adults in skilled nursing and post-acute facilities in Ossipee, NH, as part of a collaborative, team-based practice.

• Primary: Ossipee, New Hampshire

Schedule & Clinical Duties

• Full-time positions with flexible scheduling
• Typical shifts: six-hour facility rounds beginning between 7:00–10:00 a.m.
• No on-call or overnight responsibilities
• Perform daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans, and coordinate care with facility staff and families
• Efficient documentation using streamlined EHR with AI scribe, voice dictation, and company-provided iPad with LTE
• Access to UpToDate for clinical guidance

Qualifications

• Active, unrestricted Nurse Practitioner license
• National certification (ANCC, AANPCB, PNCB, AACN, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
