Attending Physician — Hospitalist (Skilled Nursing Facilities) — Sulphur, LA

Provide full-time attending physician care for residents in skilled nursing facilities across Sulphur, LA. You will provide clinical oversight, collaborate with nurse practitioners and physician assistants, and lead quality initiatives to improve outcomes for older adults. This role offers clinical autonomy without on-call or overnight duties.

Responsibilities

• Serve as attending physician for assigned skilled nursing facility residents
• Evaluate, diagnose, and manage acute and chronic medical conditions
• Develop and authorize individualized care plans
• Supervise and collaborate with NPs/PAs in a team-based care model
• Participate in Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and other quality improvement programs
• Complete timely documentation using provided EHR and documentation tools

Schedule and Work Environment

• Full-time with flexible scheduling; no fixed start/end times
• No on-call or overnight responsibilities

Qualifications

• MD or DO with active, unrestricted U.S. medical license or eligibility for licensure
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical decision-making, leadership, and communication skills
