Attending Physician — Hospitalist (Skilled Nursing Facilities) — Sulphur, LA

Full-time Attending Physician role providing patient-centered medical care to residents in skilled nursing facilities across the Sulphur, LA area.

Responsibilities

• Provide clinical oversight and serve as attending physician for assigned SNF residents.
• Collaborate with Nurse Practitioners and Physician Assistants in a team-based care model.
• Diagnose and manage acute and chronic conditions; develop individualized care plans.
• Participate in Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and quality improvement initiatives.
• Complete documentation in the EHR using voice dictation and AI-assisted scribe tools; use UpToDate® for clinical support.

Schedule and Work Environment

• Full-time with flexible scheduling; no fixed start/end times.
• No on-call or overnight responsibilities.
• Focus on outpatient/residential SNF care rather than hospital inpatient call.

Qualifications

• MD or DO with active, unrestricted U.S. medical license or eligible for licensure.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine.
