Job Description
We are seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine Physician to join a rural primary care practice in Colebrook, New Hampshire. This is an excellent opportunity to provide comprehensive care to patients in a close-knit, underserved community while enjoying a balanced lifestyle and strong community connection.
Position Highlights
- Rural outpatient primary care
- Comprehensive care for adults and/or patients of all ages

Responsibilities
- Provide comprehensive primary care, including physical exams, diagnosis, and treatment.
- Manage acute and chronic medical conditions and develop individualized care plans.
- Provide preventive care, wellness visits, immunizations, and health screenings.
- Order and follow up on diagnostic testing and communicate results to patients.
- Prescribe and manage medications and treatment plans.
- Coordinate referrals and care with specialists and other healthcare providers.
- Educate patients and families on disease prevention, nutrition, lifestyle changes, and healthy living.
- Collaborate with nurses, medical assistants, and other members of the healthcare team.
- Maintain accurate and timely medical records.
- Participate in quality improvement initiatives.
Qualifications
- MD or DO
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Completion of an approved residency program
- Current BLS and ACLS certifications through the American Heart Association
- Comfortable maintaining an independent patient panel
