Skilled Nursing Facility Hospitalist NP/PA — Cleveland, OH (Full-Time, Flexible)

Provide high-quality, patient-centered primary and acute care to residents in skilled nursing and post-acute facilities across Cleveland, OH (additional openings in FL, GA, IL, MA, MI, NH, NC, WI).

Schedule and Workload

• Full-time with flexible scheduling
• Six-hour facility shifts; rounds typically start between 7:00–10:00 a.m.
• No overnight shifts or on-call requirements
• Daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans

• Streamlined EHR with AI scribe and voice dictation

Qualifications

• Active, unrestricted Nurse Practitioner license
• National certification (ANCC, AANPCB, PNCB, or equivalent)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
