Job Description

Join a growing team dedicated to delivering patient-centered care to residents in skilled nursing facilities across Mt. Pleasant, Michigan. We’re looking for compassionate Nurse Practitioners and Physician Assistants who enjoy working in a collaborative, team-based setting and are committed to improving the health and quality of life for older adults.

Location options

  • Primary: Mt. Pleasant, Michigan
  • Additional Michigan facilities: Carson City, Charlevoix, Crystal Falls, East Lansing, Escanaba, Iron River, South Haven
  • Other states with openings: Illinois, New Hampshire, North Carolina, Ohio, Wisconsin

What the role looks like

  • Full-time positions with flexible scheduling
  • Typical full-time shift: six-hour facility visits (rounding generally starts between 7:00–10:00 a.m.)
  • No overnight shifts and no on-call duties
  • You’ll perform daily rounds, manage acute and chronic conditions, prescribe medications, and create treatment plans
  • Efficient documentation supported by a streamlined EHR with AI Scribe, voice dictation, and a company-provided iPad with LTE
  • Clinical support available via UpToDate®

Who we’re looking for

  • Active, unrestricted Nurse Practitioner or Physician Assistant license
  • Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
  • Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred
