Job Description
Provide high-quality, patient-centered care to residents in skilled nursing facilities across Mt. Pleasant, MI. This role emphasizes clinical autonomy, flexible part-time scheduling, and focused post-acute and wound care management.
Responsibilities
- Perform on-site rounds and comprehensive assessments.
- Diagnose and manage acute and chronic conditions, including wound evaluation and treatment.
- Develop individualized treatment plans and prescribe medications.
- Manage acute and emergent clinical issues on-site.
- Communicate with facility nursing staff by phone or secure messaging.
- Document care accurately and promptly using a company-issued iPad.
- Maintain reliable on-site attendance and adhere to facility and regulatory requirements.
Schedule & work environment
- Part-time with flexible scheduling.
- Typical facility shifts are ~6 hours; rounds generally begin between 7:00–10:00 a.m.
- No on-call or overnight requirements.
- Company-provided iPad with LTE for mobile charting.

Qualifications
- Active, unrestricted U.S. NP or PA license.
- Board certification required: NP — ANCC, AANPCB, PNCB, or AACN; PA — NCCPA.
- Experience in post-acute care, skilled nursing, wound care, or related settings preferred.
- Relevant clinical backgrounds include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, or Geriatrics.
- Strong clinical judgment, effective communication skills, and commitment to patient-centered care.
