Job Description

Join a growing team delivering patient-centered primary care in skilled nursing facilities across Wyoming, Michigan. This part-time role offers flexible scheduling, six-hour daytime shifts (typically starting between 7:00–10:00 a.m.), no overnight or on-call duties, and collaborative practice with physicians and facility staff to provide high-quality, evidence-based care.

Locations

  • Primary: Wyoming, MI
  • Additional Michigan sites: Bad Axe, Iron River, Mt. Pleasant
  • Nationwide opportunities: GA, IL, IN, MO, OH, TN, WI

Key responsibilities

  • Conduct daily rounds and manage acute and chronic conditions in SNF residents
  • Perform 15–20 patient visits per day on average
  • Prescribe medications, develop treatment plans, and ensure continuity of care
  • Collaborate with nursing and interdisciplinary teams to optimize outcomes
  • Maintain compliance with clinical and regulatory standards

Schedule & work environment

  • Part-time with flexible scheduling
  • Six-hour facility shifts; no call or overnight coverage

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
  • Clinical focus: Board Certified or Board Eligible in FM, IM, Geriatrics, PM&R, or ED
  • Strong clinical judgment, communication skills, and ability to work independently while engaging effectively with a multidisciplinary team
