Job Description

Provide patient-centered primary care to residents in skilled nursing facilities across Bad Axe, Michigan, as part of a growing clinical team. This part-time role offers a flexible schedule with no overnight or on-call duties and emphasizes collaborative, evidence-based care alongside physicians and facility staff.

Locations

  • Primary: Bad Axe, MI
  • Additional Michigan opportunities: Iron River, Mt. Pleasant, Wyoming
  • Nationwide openings: GA, IL, IN, MO, OH, TN, WI

What you’ll do

  • Work part-time with flexible scheduling; typical shifts are six hours, starting between 7:00–10:00 a.m.
  • See an average of 15–20 patients per day
  • Perform daily rounds, manage acute and chronic conditions, prescribe medications, and create individualized treatment plans
  • Collaborate closely with nursing staff and interdisciplinary teams
  • Maintain clinical and regulatory compliance
  • Use company-provided iPad with LTE for documentation and communication

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Nurse Practitioners: board-certified by ANCC, AANPCB, PNCB, or AACN; Physician Assistants: NCCPA certification
  • Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical judgment, communication skills, and patient-centered approach
  • Able to work independently and as part of a multidisciplinary team
