Job Description

Become part of a growing team delivering patient-centered primary care to residents in skilled nursing facilities across Ridgway, Illinois. This part-time role offers a flexible schedule, predictable daytime shifts with no overnight or on-call duties, and close collaboration with physicians and facility staff to provide high-quality, evidence-based care.

Locations

  • Primary: Ridgway, IL
  • Additional facilities in Illinois: Coulterville, Flanagan, Galesburg, Henry
  • Nationwide opportunities: GA, IN, KS, MI, MO, OH, TN, WI

What the role looks like

  • Part-time with flexible scheduling and six-hour facility shifts
  • Rounds typically start between 7:00 a.m. and 10:00 a.m.
  • No call or overnight responsibilities
  • Average 15–20 patient visits per day
  • Perform daily rounds, manage acute and chronic conditions, prescribe medications, and create treatment plans
  • Work closely with nursing staff and interdisciplinary teams to coordinate care
  • Maintain clinical and regulatory compliance
  • Company-provided iPad with LTE

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Board certification: Nurse Practitioners (ANCC, AANPCB, PNCB, or AACN); Physician Assistants (NCCPA)
  • Board certified or eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical judgment, communication, and patient-care skills
  • Comfortable working independently while collaborating effectively within a multidisciplinary team
