Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing (DeKalb, IL)

Provide patient-centered primary and acute care to residents in skilled nursing facilities across DeKalb, IL, working within a collaborative, team-based environment focused on geriatric and post-acute care.

Location

• DeKalb, Illinois (additional openings in MA, MI, NH, NC, OH, WI)

Schedule

• Full-time with flexible scheduling
• Six-hour facility shifts; rounds typically begin between 7:00–10:00 a.m.
• No call or overnight responsibilities

Core responsibilities

• Perform daily rounds in skilled nursing facilities
• Assess and manage acute and chronic conditions
• Develop and adjust treatment plans; prescribe medications
• Coordinate care with interdisciplinary teams and facility staff
• Document patient encounters efficiently using company EHR

Clinical tools and technology

Qualifications

• Active, unrestricted Nurse Practitioner license
• Board certification (ANCC, AANPCB, PNCB, AACN, or equivalent)
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
