Job Description

Join a growing team delivering patient-centered primary care to residents of skilled nursing facilities in Henry, Illinois, with additional facility opportunities in Coulterville, Flanagan, Galesburg, and Ridgway, IL, and openings nationwide (GA, IN, KS, MI, MO, OH, TN, WI). This part-time role offers a flexible schedule, no call or overnight responsibilities, and close collaboration with physicians and facility staff to provide evidence-based care.

Schedule & Clinical Duties

  • Part-time with flexible scheduling; six-hour facility shifts
  • Rounds typically start between 7:00 a.m. and 10:00 a.m.
  • No call or overnight shifts
  • Average 15–20 patient visits per day
  • Perform daily rounds, assess and manage acute and chronic conditions, prescribe medications, and develop treatment plans
  • Work closely with nursing and interdisciplinary teams to coordinate care
  • Maintain compliance with clinical and regulatory standards

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • Board certification: NPs — ANCC, AANPCB, PNCB, or AACN; PAs — NCCPA
  • Board certified or board eligible in FM, IM, Geriatrics, PM&R, or ED
  • Strong clinical judgment, communication, and patient-care skills
  • Ability to work independently and collaborate effectively within a multidisciplinary team
