Hospitalist Nurse Practitioner or Physician Assistant — Skilled Nursing/Long-Term Care (Hillsboro, IL)

Provide high-quality, patient-centered care to residents in skilled nursing and post-acute facilities in Hillsboro and Cahokia, IL. Seeking Nurse Practitioners (NPs) experienced in hospitalist, general, geriatric, or long-term care settings to join a collaborative, team-based practice. Additional openings available nationwide (MA, MI, NH, NC, OH, WI).

Responsibilities

• Conduct daily rounds in skilled nursing facilities; manage acute and chronic conditions
• Prescribe medications and develop treatment plans
• Complete efficient documentation using company EHR (AI Scribe, voice dictation) on a company-provided iPad with LTE
• Use UpToDate® for clinical decision support
• Work with interdisciplinary teams to coordinate care

Schedule and Workload

• Full-time positions with flexible scheduling
• Typical facility shifts are six hours; rounds start between 7:00 a.m. and 10:00 a.m.
• No overnight shifts or on-call requirements

Requirements

• Active, unrestricted Nurse Practitioner license
• Board certification (ANCC, AANPCB, PNCB, AACN, or equivalent)
• Skilled nursing, long-term care, geriatrics, or post-acute care experience preferred
