Hospitalist Nurse Practitioner / Physician Assistant - Skilled Nursing Facility (Temperance, MI)

We’re expanding our team in Temperance, Michigan and looking for a dedicated clinician to provide patient-centered primary care in skilled nursing facilities. This full-time role offers a flexible schedule, no overnight shifts or on-call duty, and the chance to work closely with physicians and facility staff to deliver high-quality, evidence-based care.

What the day looks like

• Six-hour facility shifts with rounds typically starting between 7:00 a.m. and 10:00 a.m.
• Average caseload of 15-20 patient visits per day
• You’ll perform daily rounds, manage acute and chronic conditions, prescribe medications, and create individualized treatment plans
• Work directly with nursing staff and the broader interdisciplinary team to coordinate care
• Maintain clinical and regulatory compliance while practicing independently within the care team

Qualifications

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
• NP board certification through ANCC, AANPCB, PNCB, or AACN; PA certification through NCCPA
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical judgment, excellent communication skills, and a commitment to compassionate patient care
• Comfortable working independently while collaborating within a multidisciplinary team
