Locums NP/PA - Hybrid Geriatric Primary Care (Bronx/Brooklyn/Ridgewood) $75-$95/hr

We’re looking for an experienced Nurse Practitioner or Physician Assistant to join a value-based primary care team serving older adults in New York. This locum position is hybrid - combining outpatient clinic shifts with home-based visits - and is ideal for clinicians who enjoy continuity of care, hands-on patient time, and working within a collaborative team.

Compensation: $75-$95 per hour

What the schedule looks like

• Full-time, guaranteed hours
• Monday-Friday with an 8:30 a.m. start
• Low daily patient census to allow thorough, patient-centered visits

Typical daily mix

• Home visits: 3-5 patients per day
• Clinic visits: 7-10 patients per day
• Locations: Bronx, Brooklyn, and Ridgewood, NY

Core responsibilities

• Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and acute problem treatment
• Diagnose, treat, prescribe, and create individualized care plans
• Order and interpret labs and imaging studies
• Accurately document all encounters in the EHR
• Educate patients and promote wellness and self-management
• Coordinate care with physicians, nurses, and support staff, and handle referrals and follow-up
• Maintain compliance with state, federal, and professional regulations

What we’re looking for

• Active NP or PA license in New York
• Minimum two years’ experience in primary care
• National board certification
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians preferred
• Expedited credentialing available
