Locum Nurse Practitioner or Physician Assistant - Hybrid Geriatric Primary Care (Brooklyn, NY)

Join a value-based primary care team serving older adults in Brooklyn. This locum opportunity is a hybrid role combining outpatient clinic coverage with home-based visits, focused on delivering coordinated, patient-centered geriatric care.

Compensation

• $75-$95 per hour

Schedule & Workload

• Full-time, guaranteed hours
• Monday-Friday (5 days/week), typical start 8:30 AM
• Low daily census
  • Clinic: ~7-10 patients/day
  • Home visits: ~3-5 patients/day

• Hybrid: outpatient clinic + home-based care
• Value-based primary care model for geriatric patients

Key Responsibilities

• Provide comprehensive primary care for older adults, including preventive care, chronic disease management, and acute care
• Diagnose, prescribe, and develop individualized treatment plans
• Order and interpret labs, imaging, and other diagnostic tests
• Accurately document care in the EHR
• Deliver patient education and support wellness initiatives
• Coordinate care with physicians, nurses, support staff, and specialists; manage referrals and follow-up
• Maintain compliance with state, federal, and professional regulations

Qualifications

• Active NP or PA license in New York
• Minimum 2 years’ primary care experience (geriatric experience preferred)
• National board certification
• Expedited credentialing required
• Medicaid and Medicare provider IDs required
