Hospitalist Nurse Practitioner or Physician Assistant - Skilled Nursing/Long-Term Care (Ayer, MA)

We’re expanding our team of clinicians who deliver compassionate, patient-centered care to residents in skilled nursing facilities across Ayer, Massachusetts. If you’re a Nurse Practitioner or Physician Assistant who enjoys working with older adults in a collaborative, team-based setting, you’ll fit right in.

What you’ll do

• Provide daily rounds at skilled nursing facilities, managing acute and chronic conditions and developing treatment and medication plans
• Complete efficient documentation using our streamlined EHR with AI Scribe and voice dictation
• Work closely with an interdisciplinary team to support resident health and quality of life
• Use UpToDate® for real-time clinical guidance

Schedule and workflow

• Full-time positions with flexible scheduling
• Six-hour facility shifts for full-time providers; rounds typically start between 7:00 a.m. and 10:00 a.m.
• No overnight responsibilities and no on-call expectations
• Company-provided iPad with LTE for clinical work

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA)
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred
