Adult Primary Care Physician - Post-Acute ISNP (Geriatrics) - Las Vegas, NV

We are seeking a full-time Adult Primary Care Physician to deliver comprehensive medical care for members enrolled in an Institutional Special Needs Plan (ISNP). In this role you will care for medically complex seniors in skilled nursing facilities, long-term care, and other institutional settings across the Las Vegas area. You will focus on proactive care management, interdisciplinary collaboration, and reducing avoidable hospitalizations to improve clinical outcomes and quality of life.

• Base salary: $211,369-$317,053

Key qualifications

• MD or DO from an accredited medical school
• Completion of an accredited residency in Family Medicine, Internal Medicine, or Geriatrics
• Board certified or board eligible
• Active Nevada medical license or ability to obtain licensure; valid DEA registration
• Preferred: two or more years’ experience in SNF, LTC, post-acute care, or complex senior care
• Preferred: experience with HCC coding, Medicare Advantage, HEDIS, and STARs documentation
• Comfortable practicing in institutional and post-acute care settings and willing to travel between facilities in Nevada
