Full-Time Family Medicine Physician - Elkhart, IN (Primary Care, Multidisciplinary Team)

Responsibilities

• Deliver comprehensive primary care across the lifespan, including diagnosis and management of acute and chronic conditions
• Provide preventive care, wellness exams, and routine health screenings
• Build long-term relationships with patients and families
• Coordinate care with specialists and the interdisciplinary team to optimize outcomes
• Apply evidence-based medicine and maintain accurate, timely documentation in the electronic medical record

Qualifications

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine
• Completion of an accredited Family Medicine residency
• Eligible for or currently hold an unrestricted Indiana medical license
• DEA certification or eligibility to obtain
• Strong clinical, communication, and interpersonal skills
