Primary Care Physician (Family/Internal Medicine) - Brooklyn Community Health System

Key responsibilities:

• Deliver high-quality outpatient primary care with an emphasis on evidence-based, patient-centered management.
• Engage in population health initiatives and care coordination to improve outcomes across panels.
• Participate in teaching and professional development activities, including mentorship of trainees and involvement in graduate medical education.
• Collaborate with multidisciplinary teams to support integrated care pathways and value-based care goals.

Compensation and benefits:

• Competitive total compensation: $180,000-$320,000 annually (base salary plus productivity incentives, depending on experience and training).

Requirements:

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Completion of an accredited residency in Internal Medicine or Family Medicine.
• Eligible for or able to obtain an unrestricted New York medical license prior to employment.
• Current BLS certification.
• Experience with Epic or a comparable enterprise EHR.
• Cultural competency and/or multilingual skills desirable for eastern Brooklyn communities (Spanish, Haitian Creole, Bengali, or other commonly spoken languages).
• Interest in value-based care, population health, care coordination, and a desire to teach and participate in graduate medical education.
