Medical Director/Attending Physician - Flexible Schedule, No Nights/Call, Mishawaka IN

Provide clinical leadership and deliver high-quality, patient-centered care for residents of skilled nursing and long-term care facilities in the Mishawaka, Indiana area. In this full-time role you will collaborate closely with experienced Nurse Practitioners and Physician Assistants to manage post-acute and chronic conditions, oversee clinical care within assigned facilities, and support continuity and quality of care across the provider team.

Key responsibilities

• Serve as the attending physician across local skilled nursing facilities, partnering with APPs to coordinate patient care and treatment plans.
• Provide clinical oversight, make medical decisions, and ensure compliance with facility protocols and regulatory standards.
• Conduct on-site visits (typically 18-25 visits per day) and participate in interdisciplinary team meetings.
• Contribute to quality improvement initiatives and communicate effectively with patients, families, and facility staff.

Qualifications

• MD or DO with current, unrestricted U.S. medical license or eligibility for licensure.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine.
• Strong clinical judgment, leadership, and communication skills.
